Log Hour Forms
For your convenience, log forms for our hands-on courses are available to download.
Important Notes:
1. The style and nature of some of the logs do not allow for direct (pre-formatted) entry. You will find these as PDF’s, only. If you do not have Adobe Acrobat Reader, you can click on the graphic below to download the free Adobe program.
2. If you wish to download logs for photocopying or to manually enter, please use the PDF files.
3. If you would like to type directly into the Giving, Receiving, and/or SOAP logs (pre-formatted), please use the WORD files. The WORD Intake and Client Questionnaire files are not formatted for entry and will print as a plain document for manual entry. You must first save the downloaded files to your computer in order to fully utilize the features of each file type. Please make sure to read the READ ME FIRST file BEFORE downloading any of the Word documents.
